For your convenience, we have outlined information about what you can expect during your first appointment with us, your rights as a patient of our practice and more. Before Your First Appointment
Please note that patients who begin treatment with Virginia Sleep & TMJ Therapy must have a referral from their physician and/or have been diagnosed with a sleep disorder. Referrals can be from sleep physicians, pulmonologists, primary care physicians or another care provider. If you are interested in starting treatment with our practice and do not yet have a referral, please talk to your doctor.
Your First Appointment
We aim to foster an office environment that is comfortable, clean, welcoming and calm. During your first appointment with us, our front desk team will get you checked in and collect any necessary information, including medical history, test results and insurance and forms. Once you have been seen by our front desk team, you will meet with one of our doctors for an initial consultation.
New Patient Forms
For your convenience, we have provided downloadable versions of our new patient information and forms below. To save time during check-in, simply download the forms, print them out at home, fill out the required information and bring them with you to your first appointment with us. Our Policies & Mission
Our mission is to deliver the highest quality of care to our patients in the same manner that we would want to be given to ourselves and our loved ones. Your scheduled appointment time has been reserved specifically for you. We request a 48-hour notice if you need to cancel your appointment. We are aware that unforeseen events sometimes require missing an appointment. After missing your second appointment without notifying us 48 hours in advance, you may be charged an additional fee.
